About the Role
Insyt FamilyCare is seeking a qualified and experienced Registered Nurse to join our team as a Patient Care Coordinator. This role is central to delivering our signature standard of concierge home healthcare, ensuring every client receives seamless, well-coordinated, and compassionate care.

Key Responsibilities
•Conduct comprehensive client assessments
•Oversee client intake, onboarding, and care planning in collaboration with the nursing team
•Assign and schedule caregivers while ensuring full regulatory compliance
•Manage daily case coordination and support hospital-to-home transitions
•Build and maintain strong client relationships through consistent follow-ups
•Ensure accurate documentation and adherence to healthcare standards
•Assist with operational reporting and drive continuous service delivery improvements

Requirements
•Must be a Registered Nurse (RN)
•Clinical experience required and home care experience preferred
